The Aron Regimen for Severe Eczema
Many of the patients who come to Eczema-doc have severe eczema. Typically they have tried several creams, often for years, and are exhausted by constant itch, broken sleep and skin that never fully clears. Severe eczema is where treating inflammation, bacteria and the skin barrier together can make the biggest difference.
Why severe eczema often responds to a combined approach
The more severe the eczema, the more likely it is to be heavily colonised by Staphylococcus aureus. In severe eczema, bacteria are often a major driver of inflammation, even when the skin does not look obviously infected. A treatment that addresses the bacteria as well as the inflammation and the barrier tackles more of what keeps severe eczema going.
How treatment is adapted for severe eczema
Stronger preparations may be prescribed for thick or long-standing areas on the body, with milder ones for the face and skin folds.
Frequent application in the early phase, to all affected skin.
Additional measures where needed, such as an antiseptic wash or a short course of oral antibiotics when infection is widespread.
Close monitoring with photographs and a POEM symptom score at each review.
A planned step-down as the skin improves.
What to expect
Every patient is different, and severe eczema usually takes longer to settle than mild eczema. Many patients notice the itch easing and sleep improving first, followed by a reduction in redness, weeping and thickened skin over the following weeks. Our reviews at two and four weeks allow us to adjust the treatment and ensure progress is on track.
When the Aron regimen may not be enough
Some people with severe eczema need treatment beyond topical therapy, such as phototherapy, oral immunosuppressants, biologic injections or JAK inhibitors. These are prescribed by dermatologists. If your eczema is not responding as expected, or your history suggests you need systemic treatment, we will tell you honestly and advise you on the next steps.
If you are already on other treatments
Please tell us about all treatments you are using, including tablets, injections and phototherapy. Do not stop any prescribed treatment without discussing it with the doctor who prescribed it.
Seek urgent care if…
your eczema suddenly becomes painful with clusters of blisters or punched-out sores, the skin is hot, swollen and spreading, or you have a fever or feel unwell.
